What CSV format do you expect?

Header line: title,page,level. Level 0 is top-level; higher numbers nest under the previous lower level, same as many outline imports.

Will this keep my original PDF features?

The tool writes a new outline (bookmarks) into your file. Other content is preserved as loaded by pdf-lib; very complex PDFs should be checked in your viewer after download.
